Human error led to 2015 Denver train crash
In April 2015 a Metro Express train collided with the back of a MetroPlus train at the Denver Train Station.
PRETORIA - An investigation into the Denver train station crash, which left one person dead and 240 injured, has found human error led to the incident.
In April last year, a Metro Express train collided with the back of a MetroPlus train that had stopped at the Denver Train Station in the south of Johannesburg.
The Railway Safety Regulator found that the Metro Express train driver was speeding and failed to adhere to two warning signals to stop.
Transport Minister Dipuo Peters says the Passenger Rail Agency of South Africa (Prasa) must implement the recommendations, to improve safety on the country's rail network.
"Risk assessments must be done on all search light signals, the signals must be replaced. Prasa must prioritise the implementation of the automated train protection system to avoid human error, which cause incidents of this nature."
