Correctional Services officials on Thursday said they are still investigating Bongani Moyo’s jailbreak bid.
On Wednesday, Moyo and two others, tried to escape from the Pretoria Central Prison.
The inmates were arrested as they tried to scale the facility's perimeter wall.
Officials said the three men sustained serious injuries.
Questions have now been raised on how Moyo and his co-accused were able to escape from one of the prison’s high security sections.
Correctional Services spokesperson Phumlani Ximiya said authorities are trying to solve the mystery.
“No one has been suspended. We are trying to determine where the weakest link was in the security chain.”
Meanwhile, Moyo was denied access to his lawyer until Monday.
He is recovering at the prison’s hospital wing.
Ximiya dismissed claims Moyo was assaulted during his recapture.
In 2011, Moyo escaped from a Boksburg prison and was apprehended at the Beitbridge Border Post in Limpopo. He was crossing into South Africa from Zimbabwe.
Later that year, he walked out of the Pretoria Magistrate’s Court, while on crutches.
